Please provide the total number of cases for 10,000 patients for the following superbugs for each calendar year starting from 2005 through 2013:

Methicillin-resistant Staphylococcus aureus (MRSA);

Vancomycin-resistant Enterococcus (VRE);

Carbapenem-resistant Enterobacteriaceae (CRE);

Klebsiella;

Acinetobacter;

Enterobacter;

Pseudomonas; and

Clostridium difficile.
